How to revert a form to the previous version?

I am not sure what happened, but it seems that somehow 2 of my forms seem to have merged together? I am not sure what has happened, I had not made any changes in about a week or so, and they were working fine, but the forms in question are my "Class A Road Exam" and "Seat Time" form.

Actually, the "Class A Road Exam Form" is the one really in question. I have made no changes to that form, as it was already completed. But I was working on the "Seat Time" form, and I know there were a lot of support folks helping me on that form, but not my other form is messed up. Is there a way to "rollback" to an earlier period to correct them, because if I have to recreate the road exam form, that is an incredible task I do not want to take on.

Can you provide some insight on what could have possibly happened? I need to fix this ASAP, because now we cannot do out testing.

I checked the revision history of your "Class A Road Exam" form and I saw that the form was edited today. You can revert it to the previous version.
