How did you embed the form on the site? I do see, upon inspection, that the form is contained within an iFrame, but it does not look like the full iFrame embed code.

If you re-embed your form using the iFrame Embed Code, does the issue persist?

Also, upon making a clone of the form in question, I see that you have integrated with PayPal: often, PayPal will redirect to collect payment, or will have a pop up on the browser. Are pop-ups and redirects enabled on your browser? This may be the cause of the issue as well.
