issue with sheets integration (not updating)

Situation: have an actual form which is integrated with google sheets and was working fine but a problem just arised when added a modification to the actual "form" and the same is not showing the updated version of the form in sheets (new "data" column add-on). Please confirm what might be the issue and how to resolve. Tks.

Including Form link below as well as screenshot of actual integrated "sheets" which doesn't have the new add-on data appearing.

Please re-integrate your form with Google Sheets.

That issue happens when you made some changes to the form or customized the integrated Sheet. These changes break the integration, so re-integration after the said actions is necessary.
