    oops! So sorry!

    I am running an online class for young kids and looking for a form for the parents that includes information regarding:

    Granting permission for the child(ren) to take this class with my company, holding us harmless for any issue that occurs when they are creating art with us


    My company's right to record and take pictures of the online class and use in our printed and online PR materials

    Code of Conduct regarding the kids: No bullying, threats, intentionally disrupting the class, etc.

    Parents would be signing their kids up for a class with an adult online...I want to avoid any issue regarding setting ourselves up for claims of untrue inappropriate behavior, etc. Sadly, we live in a litigious world....just want to be sure to cover all bases ...

    I know forms like this must be out there but I can't seem to find any.

    What you have mentioned looks like some Terms and Conditions.

    You can add the text into a Paragraph field on the form

    OR add them into a Short Scrollable Terms widget on the form: https://www.jotform.com/widgets/short-scrollable-terms

    You can also add a signature field on the form: https://www.jotform.com/help/433-how-to-add-an-e-signature-to-your-form
