forms to automatically email to me with scores tallied based on individual forms scoring

I usually like to have those forms automatically emailed to me with scores tallied based on individual forms scoring and questions have different scoring values. How do I create those?

You can add a notification on your form so that you'll receive an email whenever someone submits your form.
