Internal Sale department - Sampling form

Hi, do you know if theres a way to create a Sampling form with product and quantity?

The goal is not to only create the form but to be able to sync the information to a CSV or to have an Order URL afterwards. Then from the CSV / Order URL we want to send this data to Shopify or Quickbook commerce to create an order. We will use Zapier to send the data over.

Lets say i do create my form and fill it. Does jotform provide a Order URL when I complete it?

Otherwise, do you know any possible way for us to create such flow?

After that, you can download CSV by following this guide: https://www.jotform.com/help/73-how-to-download-form-submissions-as-excel-csv-pdf

What do you mean by order URL? Please share a sample of it and we will see if we have that option.
