Apologize for the inconvniance this may ahve caused.
I checked and your form is disabled. Please enable it in the Form Settings.

I'm glad to hear that the from is active now.
I'm not sure why this happened. Is it possible that you accidentally disabled or archived the form?

The only other possibility is that I can think of is that you used Disable on Date, Disable on Submission Limit, or Disable on Date and Submission Limit options.

If you did not use one of those options then it was most likely a glitch on our end.
