Integrate with WordPress Website

    Hello,

    We were referred to you by a colleague who uses your forms on their website. They have integrated the forms right on their site with some sort of logic based format.

    Their website is https://www.spokenbalance.com/paperwork.

    Can you please tell me how they did this and how they were able to set this up so that by answering specific questions, the appropriate form will appear?

    We would love to implement this on our website, similar to what she has done on her site, for ease of use with our clients.

    After creating your form, you can add conditions to show/hide parts of it based on user's answer.

    Thank you for this information.
    So, the initial form embedded on the website (shown below in screenshot) was a form created in jot form and certain forms appear based on the answer of these questions?

    Yes, it is a form created in JotForm.

    It is actually a one long form and its parts are hidden based on user's answer.
