Pre-sales Question: Can JotForm use pre-existing Fillable PDF Forms?

My website design client is a doctor's office and their website is built on top of WordPress and Core Gravity Plugin is installed.

Here is their new requirement: The office wants to distribute any combination of 9 fillable PDF forms to their new / current patients online, for example a HIPAA form or a Pre-Surgery form.

The doctor's office has already created these fillable pdfs and they want the PDF forms to be accessed online by the patient, filled out online, and then they want the completed form to be emailed back to the doctor's office.

Is this something that jotform can do? Also, is any HIPAA compliance built-in?

    Thank you!

    Hello Pamela,
I found your project very interesting!

I went on a search and I found out that Jotform provide compatibility with HIPAA as we can see here https://www.jotform.com/hipaa/

At the end of the page, you can create a HIPAA-compliant form.

    Hi VincentJay,

I have another question: Can your product https://www.jotform.com/products/smart-pdf-forms/ be integrated with WordPress? I looked at the WordPress plugin and it doesn't seem to be used a lot. Please let me know if I am looking at the correct plugin: https://wordpress.org/support/plugin/embed-form/reviews/

    Thank you for your help.

    Hello,

Thank you for coming back to us. Yes, you can embed your form to WordPress using WordPress plugins. You could also simply embed your form using iframe embed code as well. To get the iframe code, please click "PUBLISH" at the top of your screen and click "EMBED" on the left hand side panel. You should be able to generate "iframe" code from there.
