Q: How come it says from Australia even though the form clearly says MALAYSIA?
A: Upon investigation, it is possible that you've hard-coded the word "AUSTRALIA" in the autoresponder's body? Please refer to the screenshot below:
In any case, you can simply just replace it with MALAYSIA. Click on Settings -> Email -> Click the pencil icon of the autoresponder. Scroll down to the bottom of the email's body and edit it. Don't forget to click Save. Screencast below:

Thanks for your reply.
I've managed to change the issue already. However, I don't remember hard coding the text in as I've only inserted the text using the widget.
Anyways, I've got a new question:
How can I only send the fields that the customers answer to them in the autoresponder email?
E.g. A client has a choice to make an appointment at location A or B, & she chose location A. Can the email only show the time & date selected for location A?

We are glad to know that you have successfully updated the email. It is possible that when you first created the email, it may have been using Australia in the widget.
