My form does not appear when embedded on my website

It's a form cache problem. I've cleared that for your account. If you happen to experience this again in the future, you can clear it through your account. Here's our user guide for that.

Note that the Smart Embed is also enabled for this form. You may want to disable it to have the original form styling.
