I received 2 image and both go to a page that says “whoops”

    The issue could be related to your account privacy setting, which requires you to be logged in to your Jotform account to view uploaded files. I would suggest you to please uncheck "Require log-in to view uploaded files." privacy from your account setting page and that should solve your problem. Please check the screenshot below:
