in the workflow are real emails so please don't run any tests prior to
letting me know).

Hi again,
Checking the Approval Workflow, I can see two separated email elements as shown below. Please remove or connect them to the flow.

Also, can you please specify with a screenshot which email alerts do not work, as you have quite several?

If you mean by doctors approvers, then yes, they will, if the conditional logic is satisfied as shown below.
