Users have trouble loading forms

    Lately we have had multiple patients complain about not being able to access the jotform to fill out required new patient paperwork. We email all new patients a welcoming letter with the link provided but patients complain that they are blank forms, or that the link itself will not load? It has become extremely more frequent since the HIPPA update.

    I've cleared your form cache - this could help with the forms loading blank or not loading at all.

    Is it the form itself that loads blank, or is it a PDF document, an email, or anything of that sort?

    Does this problem happen with every one of your forms, or is it a single form that seems to be causing trouble?

    If it's just a single form, please specify which form is the problematic one - we will examine it for anything that could cause issues with loading.
