PDF Report: Conditions not showing on the report

Hi,

I did conditional logic to have the patient name show on the top of each page of the form which it is working perfectly when I test the form out. The problem is when I go to print out the pdf that is emailed to me the patient's name is only at the top of the first page NOT each page. How can get the pdf to show the patients name at the top of each page?

May I ask you to try to add the Name field on each page in the PDF Report Builder?
When you click "Add Element" on the left, you should see Form Fields options, where you can choose the Name field to be added on each page.
