Hi there,
I have created a form which holds my client to the waiver legally. I want all options to appear on the submissions once the form has been completed to keep record as a legal document. For some reason the questions for single choice are not appearing on the submission template. How do I make sure all the questions show up in the submissions template once the client has sent through the form.

Greetings,
If you mean with your question the submission template received my email then you need to disable the Hide Empty Fields option from the Advanced tab of your email notification/autoresponder settings as in the below screenshot.
Also you can get around this issue by setting all the form fields to be required, please check this guide for more details.
