I need help finding this form that I created

This is the 3rd email I have sent to JotForm for help. When I try to sign in to see this form I created it says my free account has been deleted. FORM NAME: Kalamazoo Community Foundation: Baseline Board Assessment to Advance Racial, Social, and Economic Justice.

Since this unfortunate incident, I have purchased a bronze membership but this form is not in that acct. because I made it in a free acct. I really need the info from the responses to the form listed above - can you help?
