    Hi,

    This is a temporary glitch from jot form. It will fix ASAP.

    500 Internal Server Error:  server error response code indicates that the server encountered an unexpected condition that prevented it from fulfilling the request.

    Please clear your form cache this way:

    https://www.jotform.com/help/229-how-to-clear-your-form-cache

    Could you please check and let me know how it goes.
