how to populate info based on text entry

Hi I have a form for training skills check. When I enter the same employees name in a field I want it to prepopulate based on the last time the form was used to see if they have improved and modify the assessment.

Unfortunately it's not possible to lookup directly in the submission table with an input from the form, it's possible to provide/share a prefilled form with your respondents but this way you will create a predefined link of the form for each respondent, please check this guide for more details.

Otherwise you can use Spreadsheet to Form widget in your form which enables you to lookup for a data in a spreadsheet, the downside with this option is that the spreadsheet is uploaded to the widget which means that the data source for the lookup is static and can't be updated or changed unless you upload another spreadsheet to the widget, please check the below guide to learn more about this option.
