You can put the same link on different pages. Also, you can embed the form on each page.

You can access the submissions via the Tables page:

Let us know if you have any further queries.

    Thanks.

I know I can put the same form to different pages with embed code. However, is there any way I can see the submission comes from which page?

    Hello,

Based on one unique form, I do not think so.

As the form will be embedded, he won't be aware of his surroundings basically.

The only way to do that would be to use google analytics or others on your website itself to check for the actions taken on the page.

    Best regards

Additionally, you can try to embed these widgets on the form to capture the Source of the URL:

    https://www.jotform.com/widgets/get-referrer

    Thanks.
