As you're using a WordPress site, please remove all of the form code and follow the directions provided in our Adding Forms to WordPress guide.
Do let us know if this helps.

Upon checking your web page, I found that you are using your form's source code to embed your form. I would suggest you to please embed your form using its iFrame embed code and that should solve your problem.
Please follow the steps of the following guide "Adding Forms to WordPress" and only change you need to make is to copy the iFrame embed code without the script code instead of the "Wordpress" embed code, Please check the screenshot below:
Tha following guide should help you grab your form's iFrame embed code: https://www.jotform.com/help/148-Getting-the-Form-iFrame-Code
Please use the following iFrame code to embed your form and that should help you:
<iframe id="JotFormIFrame" onload="window.parent.scrollTo(0,0)" allowtransparency="true" src="//www.jotform.co/form/32470418153853" frameborder="0" style="width:100%; height:11473px; border:none;" scrolling="no"></iframe>
Hope this helps.

Another issue I realized that your account is over quota. Free account has 100 MB of upload space allocation and you have already utilized 806.57 MB.
You should upgrade your account or delete the submission which has file attached.
