Complicated question about creating an attendance form

I have read every article I can find, but I would like to know if I can create a form (i.e. simple present/absent attendance form) from the data in a table. Every quarter I use a form to take registration for a youth athletic program (from 50 to 150 registrations), and I would like to be able to have the name of each child automatically added to an attendance form that my coaches can easily access (my plan is to provide them with an app). The reason I want this to be automated is because we allow parents to register throughout the program -- no deadline, so I do not want to have to remember to add new members to the attendance sheet. At this point, it seems like maybe the only way to do this is to have the coaches use the table to take attendance, but that is bulky. It would be great if that table could shoot data to a form, rather than just vice-versa.

Thanks for the reply, but I already know how to create tables from form data. I am wondering if I can do the opposite -- create a form from table data.

Unfortunately, it is currently not possible to automatically create a form from Table data. You would need to manually create a form and then import the data for it.

You may utilize a unique field to populate the user's data on the form if you have the user's data in an Excel spreadsheet. The Spreadsheet to Form widget allows you to do this. Please see the link below for a tutorial that can assist you with this.
