How to send out the form for users to fill out.

I’m having troubles with my consent form when I don’t know how to send it to people so they can fill it out. And if I do send it out all they can see in it is what I send the picture it can someone help me out? I’m using an iPhone if that matters

So this is my consent form don’t know if you can see it or not but I’m having troubles. I tried to fix something on it and it didn’t save the new edit. And when I try to send it no one can see what I’m sending

It seems that the form you are referring to has been deleted and now sits in the Trash folder:

If you still want to use the form, simply RESTORE it to reinstate it.
