Hi,
I did a test submission on the cloned form and I was able to replicate the error. It seems that the images you have added to the email body are corrupted.
This can be fixed by deleting the images from the email body and re-adding them.
First, you need to edit the email from the approval flows.
Use your backspace key to delete the images then click on "Insert Image" on the top menu and insert the same images. Don't forget to click the Save button. You can use the "Toggle Screen" option in the upper right corner if you need to drag to resize the image.
Do it on each email template for each Approval Decision email template and after saving it, you can do another test submission and the images should load.
