Can I create a customizable form for my website??

I am here to know that can I create a customizable form for my website Melbourne osteopathy center? My website is related to medical services and the form should be user friendly for my clients. Please let me know how can I do it?

Thank you for reaching out to Jotform Support. Yes, it is possible to customize your form as you like. To do this, please follow the steps and screencasts below:

To create your first form, please follow the steps and screencast below:

1. On your My Forms page, click on Create Form at the top-left of the page.
2. In the Create Form menu, click on Start From Scratch.
3. Next, click on Classic Form.
4. In Form Builder, click on the Add Form Element+ menu and click on the fields you'd like to use on your form.

To customize your form, please follow the steps and screencast below:

1. In Form Builder, click on the Form Designer button on the left side of the screen.
2. In the Form Designer menu, there are several options to customize your form. To change the colors of your form, in the Colors tab, click on a color scheme you'd like to use.
3. To change your Submit button, click on the Styles tab, scroll down and click on Change Button Style.
4. If you'd like to change the theme of your form, click on the Themes tab, hover your mouse over the theme you'd like to use and click on Use Theme.

After your form is created, to share your form, please follow the steps and screencast below:

1. In Form Builder, in the orange navigation bar at the top of the page, click on Publish.
2. In the Publish tab, in the Quick Share menu, click on the Copy Link button and send the link via the method of your preference (email, messaging, etc.).
