Trying to create an office supply request form

    Creating an office/medical supply request form.

    I need help with how directors can input a number amount of quantity for a product/item, assign certain items with a max amount that each department is allowed to order. I'd like to include a "summary" page for them to view before submitting.

    By a certain date, once all requests are submitted, I'd like a total of each item requested. So it is easy to total when placing orders.

    By the beginning of the month, I'd like the totals to reset. I'd like the previous months requests saved in archive for data purposes.

    Hello kristinv712,

    Thank you for reaching out to Jotform Support. You can add an input table element to your form where the Directors can input a number amount of quantity for a product/item and assign certain items with a max amount that each department is allowed to order. You can add the Review Before Submit widget for the directors to review before submitting.

    To make a total of all the items requested, it is possible to perform calculations on your Jotform Tables. Please check number 2 on this guide: 7 advanced Jotform Tables features to do more with your form data.

    To reset your entries on your Jotform Tables and saved previous submissions, you can archive submission entries by following this guide: How to Archive Submission Entries in Jotform Tables.

    How do you assign each with a max amount?

    Hi Kristin,

    Unfortunately, the number field doesn't provide us with an option to set a max count but the dropdown field as it gives us the option to enter the dropdown choices. Please see the screencast below and see if that works for you or not:

    What if there are different maximums for items listed?

    Hi Kristin,

    Actually, in Input table we could not use different quantity maximum for each dropdown. For multiple product inventory you can take a look at our Inventory Widget. Using inventory widget you can set the different quantity maximum for each product.
