Form submission emails have template name instead of my form name

Our summer camp template form is Entitiled July 5/6 Canmore Trip Registrations but when I receive the emails of people who have registered, the email is entitled "Summer Camp Registration 3rd-5th Graders.

I certainly hope that no one else that registers using our form sees this name.  How can we get that changed?  I have gone through everything and can't see anywhere where that name is still active

    Hello dirtgirls,

I did check your form but could not find any issue with that. It should be sending the submission emails with the subject line "July 5/6 Canmore Trip Registration". Please check the screenshot below:

I did check the subject line of the auto-responder email alert you have in your form and it is configured correctly as "DG Mountain Biking July 5/6 Canmore Trip".

I believe you have already resolved this issue and your form is working as expected. Do get back to us if the issue persists.

    Thank you!
