I was able to reproduce the issue in Google Chrome and Internet Explorer browsers. The issue seems to be caused by the Terms & Conditions widget on JS embed version of the form. A bug report ticket has been attached to this thread and escalated to our Development Team.
In the meantime, you can replace your form embed code with an iFrame version on the website. The iFrame version will not be affected by submission bug.
We will get back to you once we have any news on this.
