We do not allow to collect sensitive information in our form such as credit cards or to simulate a fake  form showing logos from any Bank to collect info, all this is considered a phishing activity.

Otherwise your account might get automatically suspended. Please read our terms of use. for more info.

If this is not the case, then please provide more details of the purposes of the form and we will gladly assist you.

    PLease look at this form.

    Hi Jeanette,

We are an electricity retailer and use JotForm to provide a sign up form which sits on our website - people coming to us to use it - not us sending out emails - please see our site here:

    http://www.switchclubpower.co.nz/?page=football/join_football

We are asking for bank account details not credit cards. After being veted by our bank we have been given the authority to set up Direct Debits. We are simply wanting to upgrade our form to allow our customers to do this online, which has also been approved by our bank.

    Can Jotform help us here?

    Regards,

    Hi Chris,

I checked the form on the website  but I was not able go to the next page of the form. The Next button is not clickable.

It seems there is a script conflict between your form and those existing scripts in your website that is causing the problem. To fix this, I suggest you try re-embed the form using its iframe embed code.

Follow this user guide Getting the form Iframe Code

Check if this fixes the script conflict. Inform us if it does not.

    As for this..

    12  1234  1234567  123

The spaces represent spereate boxes but we want to be lined up horizontally.

You can use the texbox field and use Masked input to indicate the specific format the input should be.
