ADD form field to PDF filename

    Hi there, the only fields displaying for my form in the PDF File name for selection are submission id and form title. How do I get the other fields in my form to populate so that I can change the name of the PDF? Thank you in advance.

    Hi there,

    Thank you for reaching out to Jotform Support. In the PDF Filename section, click on the Form Fields to select which form field value will be added to the PDF filename.

    For an instance, you've selected the name and email address from the Form Fields to fetch the PDF filename. If the responder will fill up and submits the form, the PDF filename would be the combination of the name and email address of the responder.

    If you want to change or add more fields to be included in the PDF filename, go to the same settings below and click on the Form Field dropdown menu to select fields. To remove, just hover on the field and click the "x" remove icon.

    Thank you for the quick response. I am wanting to change or add fields to be included in the PDF filename but when I click on the Form Field dropdown menu, the only fields available to select are Form Title and Submission Id. I am wanting to include last name and date of completion (these are already fields in my form). Is there a field property I am missing in order to display it in this dropdown?

    Thank you for getting in touch. The HIPAA is enabled in your account and that is the reason why it is restricting you to add more fields. The HIPAA security feature prevents any field values to show up in public. However, you can disable the PHI on each field that you want to in the PDF filename as shown below.

    PHI is set to OFF for the Name field and it shows in the Form Field dropdown.

    Please note that if this is set to OFF, it means that the value collected from the said field is not protected.

    • PHI (Protected) – means the data collected with this field is sensitive healthcare data and cannot be used in an insecure medium (like Autoresponder or Notification emails).
    • OFF (Not Protected) – means the data collected with this field can be used in an insecure medium.
