Should I create one or two jotforms?

    Good morning!

    I have a question concerning a feedback report form which was requested to be turned into a jotform. The form is attached. The top part, before For QUI Use Only, is already filled out. Being the bottom portion can only be filled out by our company personnel, is there a way to make only one jotform or is a feedback and evaluation form needing to be made?

    I understand that you want to use one form to shared by two persons. It is possible to have single form that can be shared as feedback and evaluation forms. For example, please take a look at this thread. In this example, we have email ID as the key.

    In the same way, we can set an autoresponder to the person who is evaluating the feedback report. Also, you may hide the evaluation form for the users who gives feedback report based on conditions set to a mandatory field on the form.
