Hi
For some reason the fields are invisible on the auto-responder set-up. The same happens when setting up integrations. I have tried Firefox, Chrome and Safari, same on all 3.

Hi,
I think that is happening because you have not properly assigned field label/names on the fields in your form http://www.jotformpro.com/form/42157032493956
I suggest not to let the fields without a label/names at it will most likely will cause issue on functionality of the form.
I test this on my end and I add a name to the field. When I add the email notifier I could see the field name available.
If you like the field names to be hidden in the form, you can use custom injected CSS code instead to do it.
For example in your form you can inject CSS code like this
#label_8{visibility:hidden;}
to hide the Email field name/label.
