Hello Ernest,
Thank you for waiting, I appreciate it. Can you share with us more information on why you need to add Conditions to your form? We would like to know what you are trying to achieve. Please see this link to understand the Approval Flow and this is another link for your guide in adding Conditional logic.
We can move forward with a solution as soon as we receive a response from you.

Hi Ernest,
Thank for getting back to us. The conditions can be added to the form by going to Settings and then Conditions. Let me walk you through how to add a condition to your form:
- In Form Builder, in the orange navigation bar at the top of the screen, click on Settings.
- Click on Conditions on the left side of the screen.
- Choose the conditions you would like to add on your form as per your requirements.
- Add conditions in the form.
