How much e-mail address attached to a form?

I am not sure if I have understood your question correctly. Do you mean to ask how many email address you can add in your form to receive submission emails?

Please be noted that you can only add up to 10 email address in the recipient email field of your notification email alert. 

It is also possible to add any number of email alerts in your form. To add more email address, you should add new email alert and the following guide should help you:  http://www.jotform.com/help/25-Setting-Up-Email-Notifications
