my full form is not displaying

when you select yes, additional options and the submit button are not properly displaying -- help!

The credentials to access your page are not working. However, upon checking the stand alone version of the form, I suspect there might be a conflict in the page, because the stand alone version works with no problem. My recommendation would be to embed the iFrame version of the form to prevent any conflict.
