i want to create a professional form for Crazy Minerals

Thanks for reaching out to Jotform Support. I understand that you want to create a form. It’s easy to create your own form. Here’s how to do it:

• On your My Forms page, click on Create Form at the top-left side of the page. 

• Now, click on the Use Template button to choose a template.

• Type the keywords of the template you want to use in the Search Bar, then click on the Magnifying Glass icon the right side of the Search Bar.

• After that, you can choose a template you want to use by clicking on the Use Template button at the bottom of the template picture.

• On the Form Build page, click on Add Form Element at the top-left side of the screen to add a new Form Element + menu.
• On the left side menu, click on the elements you want to add to your template. The element will appear on the form on the bottom side of your form.

• Click on the field that you want to edit, then click on the Gear icon to edit your element. That's it.
