HIPAA locked form is still unavailable

    This is extremely important to our intake process and its been down for days.

    Hi Wanda,

    Thanks for reaching out to Jotform support. I assume you're referring to this form of yours. The form has been disabled as it was flagged for HIPAA Compliance. If you don't require HIPAA let us know and we can forward that to the HIPAA team to let them know. If you do need HIPAA you need to have a Silver or higher subscription to enable HIPAA compliance on your account.

    I dont need HIPPA If I need to remove something like an input from the forms I will. Let me know what I need to remove.

    Hi Wanda,

    Thanks for getting back to us. I've checked out your two disabled forms, and it seems your forms have been set up to collect the PHI information. That's why HIPAA feature was suggested by our team. I'll still check with our relevant team and get back to you with an update on this.

    Hi Wanda,

    Thanks for patiently waiting. I just heard back from our relevant team. Since you mentioned that you don't need HIPAA, you can choose the NO option when you get asked if you are a HIPAA-covered entity by our system. Let me walk you through the steps below:

    1. Log into your Jotform account.
    2. Click on the Go Back button when you are asked 'Enable HIPAA compliance feature for your account'.
