Checkbox is not showing on the PDF

Another issue I'm having is I updated another form (Clover station duo Apogee) to include the long lists of checkboxes which does show up on jotform itself when I got to edit. However, when the form is filled out, the check marks disappear. I think it's worth noting that the form I uploaded was a flattened pdf.

Thanks for patiently waiting. I assumed you were referring to this form, I cloned your form and submitted a test submission, but I can't seem to replicate the problem on my end, the checkbox displays the answer on the PDF. If you want to see all of the options in the check box on your uploaded PDF regardless of whether they are selected or not on the submission page. Unfortunately, this is not possible because the submission page only displays the response/data that the user has submitted.

If I misunderstood you, please provide a more detailed explanation of the problem, as well as the URL of the form in question. You may also attach a screenshot, which will greatly assist us in identifying the problem.
