Consent Forms & Intake Forms

To Whom It May Concern:

I have consent forms, intake forms, waivers, etc. ready to be downloaded to any site for client signature. I was wondering if through Jotform I'm able to have these forms signed in person through my tablet at their appointment and if I'm able to create a folder for each client through your site? I saw the option to have it emailed to each client but I would rather have them sign these forms in person prior to their appointment. Please let me know if this is a feature that you offer or not at your earliest convenience. Thank you and I look forward to hearing from you soon!

Thanks for reaching out to Jotform Support. If your form is going to be filled in rapid succession, I'd recommend you use our Kiosk Mode through the Jotform App from your tablet. If not, you can just open the form through the App and hand them the tablet.

You can create folders, but for Forms, not submitters. Though you can use the filter option to filter multiple columns. That way you can filter the columns by the submitter's name. Let me show you how:

• Open your submissions in Tables, and on the top-left side of the page, next to the Search box, click on Filter.
• In the window that opens, you can add filters to your search by clicking on Add New Filter.
• After you're done, click on Apply Filter.
