A box in the build is not showing up in publish

I am raying o build a simple suggestion form- but the actual suggestion box will not show up when I go to publish. It is there in the build and preview. Please assist.

To fix this, you need to delete or fix all these conditions. To do that, you should follow the tasks below;

1. Go to the SETTINGS
2. Select CONDITIONS.
3. Click on each condition and select either the pen icon to edit the condition and fix it or the gear icon to delete, disable or clone it:
