If you wish to upgrade to Enterprise, you can fill out the Enterprise Application Form and one of our Sales Representatives will contact you to discuss more information about the pricing. You can also check the Jotform Enterprise Pricing to see the pricing for the Enterprise plan.

I have just tried accessing the Enterprise Application Form and I was able to access it with no issue. See the screenshot below:
Can you please try accessing it using a different browser or in incognito mode? Give it a try and let us know if you need any other help.
