Hi awkwordness,
Thanks for reaching out to Jotform Support. You can prevent duplicate submissions based on a certain question. Let's go through it together.
First, click on the Settings tab in the top menu of your form builder and then click on the Show More Options button:
Scroll down until you reach the Unique Question dropdown. From here, you can select the form field (ie. the Email field) that will be checked and the form will not accept duplicate submissions based on previously entered values in the select form field:

What if i'd like to be a combination of answers?
Example: someone can submit multiple times using the same email but not for the same appointment.

Thanks for getting back to us. I'm afraid we don't have this feature yet, unfortunately, I can't also think of possible workaround that can help in achieving your requirement.
