Is it possible to have multiple QR codes associated with a MS Form?

    Is it possible to have multiple QR codes associated with a MS Form? For example, I want to keep the same form but have one for the Northeast to complete and one for the Midwest to complete. So the form could route to the correct person at that location and not just to the owner of the form.
