By default, our forms only allow for a single payment field per form.  To add additional payment fields, you would need to use the workaround described here:

http://www.jotform.com/help/182-How-to-Create-a-Basic-Multiple-Payment-form

If you are trying to add the ability to collect credit card information in the purchase order, this is unfortunately not currently possible.
