How to Add Form on a Facebook Page

Dear Jotform, really interested in your service and i have succesfully make a form. However, I cant manage to make the form appear in my facebook page. Please help !

Can you also record your screen on how you tried to add your form to the Facebook Page using Loom then share it with us?
