Is there a way to correct the date on the forms so it is current? When I receive the forms the date is very out of date and does not match the date and time on the email. Is that a server issue or a form setting? Thank you.

My settings are the same as in the image you sent. I do not understand how this answers my question - could you clarify please? Thank you.

We apologize for the confusion.
I was able to see the issue you mentioned when I checked the embedded form on your website.
the issue is being caused by an outdated embedded form on the website. Since the embed code used was the full source code of the form, it needs to be updated every time there are updates/modification done on the actual form http://www.jotformpro.com/form/41895363226965
I suggest you re-embed the form http://www.jotformpro.com/form/41895363226965 on your website to be able to see the updated version of the form
I also recommend using its iframe code embed instead of the full source embed.
