I would really like to speak with someone over the phone. I have invested time and money into switching over from form central to jot form and I would really like to ask someone thequestions I have regarding e-mailing my survey through constant contact and other questions I have regarding jot form.

As to your current concern, I checked your jotform account/username deltaartsalliance but I did not find any issue with the account. It is Active and in good standings.
When I checked your My Forms page, I was able to see your forms available.
Can you tell us the actual name of the missing form? We will search for it if it is missing on your My Forms list.
