If I understood you correctly you want to import Fillable PDF form from Adobe FormsCentral to JotForm.
http://www.jotform.com/adobe-formscentral-alternative/
Choose "PDF Only" after clicking Import Forms button.

I would recommend trying the import once again.

You can also import your form and responses and then create fillable PDF form from your imported form.
