    When the form is embedded on my webpage or when doing a preview of the form only half of the form appears. This is also true when viewing the form on my webpage. I am so tired of trying to learn to use JotForm

    I see the problem when I checked the form on your website.

    It was due to script conflict.

    Please remove the currently embedded form http://www.jotform.us/form/52006689439160 on the website, and then re-embed it again using instead its Iframe embed code.
