My logo is in one of my folders on my desktop.
How do I add that image/logo to my letter?

I looked at the form you have provided us with a link to and I see it already has a logo on it.
Are you maybe referring to adding a logo into the emails that are sent from the form? Or?
Can you please explain more about what you mean by "mail merge/letter" so that we can better assist you?

Yes, I am not able to add my logo and signature to the email that goes out to the person filling out the form.

In the body of the email you have a WYSIWIG editor. This editor has a tool for insert image. The image will have to be hosted elsewhere on the internet and then take the link to the image and use it with the insert image button.
